Are you picking up a child early from school? Please complete this form at least the day before the early dismissal to let the school know your plan. It must be completed by a parent or guardian.

Your completed form will go to the school office and be forwarded to your child's teacher(s).

Instructions for early dismissal pick-up: The person picking up the child(ren) should come to the welcome desk inside the canopy doors with their cell phone (we use a QR code to access the sign-out form). The student will wait in the school office until the welcome desk calls to let the office know someone is here for them.
