Update: You can now enter all of your children on ONE form.

This form is to be completed by a parent or guardian to let the school know ahead of time if your child(ren) will be coming in late (so we don't call or email you looking for the children).

This completed form will go to our health/attendance office, school office and be forwarded to your child's teacher(s).

IMPORTANT! When bringing your student(s) to school after the school day has begun, please follow these instructions: Please walk your child in to the Welcome Desk (Door F - canopy doors) where you sign them in using the QR code (the one at the welcome desk) on your phone. Your child will receive a pass and go directly to their class.
